Referrals & Assessments Manager
Liverpool & Cheshire
£25,000 pa + £7200 OTE
Our client a leading National Care Provider is currently looking for Referrals & Assessment Manager for Liverpool & Cheshire area.
This new and exciting role will be to establish new areas of business for the organisation and to drive development in high growth areas.
Reporting to the Development Manager, you will be instrumental in working to maintain and establish new social worker and care manager relationships and develop a robust referrals pipeline in line with area sales targets for new business.
You will be instrumental in providing a positive and seamless journey for people requiring support. Also, communicating with families, care managers and health professionals to offer reassurance and pro active management to put in place a holistic package of support.
You will need to be a self-starter. You will be required to undertake assessments to an excellent standard, propose service hours and produce detailed, outcome-focused support plans and risk assessments. Working alongside operational area colleagues, you will lead the implementation ensuring a robust and person-centred focus to all systems and processes.
You will need to have experience in developing new business and strong operational expertise within the Learning Disability, Mental Health and/or other vulnerable group sectors; a professional qualification/NVQ Level 4 or Professional qualification.
Travel will be required as part of the role.
